I give my full consent as a legal adult to receive ThetaHealing® and / or any other holistic approaches as part of my participation in sessions, meditations, workshops or seminars with Alison (“Alice”) Webb at Embrace Your New Reality LLC. I understand that the results may vary and that the practitioner may not guarantee results. I understand that complementary approaches are not a replacement for medical treatment and psychological or psychiatric services. I understand I should not stop or change medication or medical treatment without the advice of a physician or licensed professional, and that I am advised to seek the care of a licensed professional for significant physical, psychological or psychiatric concerns.  I also understand that the practitioner does not treat, prescribe for or diagnose any condition. I understand the services provided by Alison (“Alice”) Webb at Embrace Your New Reality LLC are not regulated by, nor provided under, a professional license. I understand that if I choose to cancel any medical or psychological treatments or medications, I assume responsibility for any negative outcome.

I acknowledge that I am free to terminate any or all sessions, meditations, workshops or seminars at any given time. I agree to provide accurate background information when requested by the practitioner. I understand that sessions are non-refundable, but may be rescheduled up to 24 hours in advance for another available session within 30 days of the original appointment.
